Located at top left of firewall in front of cowl
The body name plate includes Production Date, Paint Color, Top Color, Interior Trim Code, and accessory codes.

    
Production Date Code - Upper Left 2 Numbers followed by a Letter
2 Digit # indicates month, and letter indicates week of month
For Name Plate Above 03C indiates it was produced in 03 = March, C=3rd week

 
Style - 2 digit year followed by 4 digit body style
example above 64-6357 - 1964 Coupe DeVille

 
Body Style Production Number on Body Name Plate
Indicated production number for that particular body style

Example above BODYFW22388: Body by Fleetwood, and 22,388th Coupe DeVille produced out of 38,195 produced

Below is a list of all 1963/64 Accessory Codes. Note that only "Body" options appear on the body name Plate. See more on Accessory Codes at bottom. 

B(1964) - rear window defogger
C - cruise control
D - door guards
E - soft ray tinted glass
F-  am/fm radio
FP - filler plate
G - controlled differential
J(1963) - rear radio control (75 series)
J(1964) - twilight sentinel (new for 64) 
K - air conditioner
L8 (1963) - license frame, one
L9 (1963) - license frame, two
8 (1964) - license frame, one
9 (1964) - license frame, two
M - power door locks
N - power vent windows
P - optional gear ratio
R - am radio
R2(1964) - rear radio control (75 series)
Q - tilt steering wheel
S - power trunk control
T - 4 way tilt bucket seat (driver side only)
U - guidematic headlight control
V(1963) - front and rear seat belts
V1(1963) - front seatbelts

Example Body Name Plates:

1963 Park Avenue with tinted glass, bucket seats, power door locks, AC, front seat belts.

1964 Four Window Sedan de Ville with tinted glass, power vent windows, 6 way power seats, power trunk, power door locks, AC, rear window defogger, front and rear seat belts.

MORE ON BODY NAME PLATE ACCESSORY CODES:

The Body Name Plate only lists optional accessories defined as "body" options, but not other accessories. As an example: my 1964 Coupe de Ville the Build List includes: FVREYDKU, but only EYKV on the Body Name Plate (see pictures of both inserted at bottom).

The way I read it is options that require different body adaptions for that option. AC would be a good example. There are many options you would never see on the "Body Name Plate" The controlled differential (Option Code G) would be one that comes to mind. You also never see radio codes (R, or F, or FP) as I believe the dash assembly with ordered radio option, would be installed into the body, without any consideration of the car body, thus, nothing on the body name plate. Way back I thought every option on a car would be listed or noted somehow in an order group but that is not the case. I have started going th rough my 54 parts cars (and 5 drivers where I have the body plate) to try and figure out exactly which codes to appear and those that do not but have never gotten very far. What I probably can do is just list out every body code I have seen on the body name plates (I have saved them from the parts cars).

I was cleaning up a couple of the body plates from parts cars today and noticed they had some not too common options, at least for these body styles. Here they are:

White 1964 Coupe de Ville, black vinyl top,
black interior, front seat belts, and bucket seats

White 1964 Four Window Series 62 Sedan with midnight blue interior, power windows, AC, front seat belts, power seats, and the bit rare rear window defogger.
